Facelift Under Local Anesthesia Newport Beach

What is a DeNovo Lift?

The Denovo lift is a hybrid facelift procedure combining the safety of local anesthesia with advanced laser technology. It is customized to each individual and when done in combination with other rejuvenation procedures, provides a graduated approach to facial rejuvenation. As an alternative to the traditional facelift, the procedure improves jowling, neck laxity and wrinkles in a less invasive manner for less downtime wi a natural result.

Traditional Facelifts

Over the years, the traditional facelift procedure has evolved into what is now known as a deep plane facelift. In an attempt to more powerfully lift the facial tissues, the operation now incorporates fairly risky techniques with deep dissections around the nerves that control the movements of the face. When combined with a traditional browlift, the incisions may go across the top of the forehead, continue in front of the ears and into the hair behind the ears altering the natural hairline in the process. The recovery of this type of procedure usually takes several weeks and may result in an unnatural “wind swept” appearance.

Face lifts and Neck Lifts have been traditionally performed under general anesthesia or IV sedation to prevent pain and movement during the procedure. Although this approach is relatively safe, there is still associated risk that could be life threatening when it comes to maintaining the airway. A ventilator is often utilized with careful monitoring by an anesthesia specialist because your spontaneous reflexes are reduced. Patients may be immobilized for several hours, often increasing their risk for blood clots. There are also other potential side effects from the often complex use of medications. Previously, patients who feared these risks had no alternative for having cosmetic surgery of the face.

Facelifts under Local Anesthesia – A Safer Alternative

In recent years, “mini-facelift” procedures have seen popularity in the office setting. Most of these procedures involve smaller incisions with limited tightening in the front of the ears. While these procedures provide an adequate results for the right candidate, they often provide minimal results for fullness or significant laxity in the front of the neck. The best candidates for good results are either younger patients without significant neck concerns or a patient desiring a refresher for a previous facelift.

More than 10 years ago, Board Certified Facial Plastic Surgeon Dr. Stephen Fink started doing minilift procedures in the office under local anesthesia. Over the course of thousands of procedures, Dr. Fink expanded and adapted the procedure to address the lower face and the neck. This procedure meets the needs of a much broader range of patient conditions. Using only oral sedation and local anesthesia, there is minimal discomfort, pain and associated risks compared with deeper anesthesia. The DeNovo lift is a synthesis of proven techniques including laser liposuction. At the start of the procedure, a laser fiber is gently passed right beneath the skin in several directions to help disrupt fat cells, lessen bleeding and shrink the skin. This helps reduce bruising and provides additional tightening and thickening of the skin as it stimulates the growth of new collagen. Traditional liposuction is then performed to the desired contour followed by a corset style platysmaplasty. Loose muscle bands are firmed in the midline and then lifted in a natural direction on each side. Finally, excess skin is mobilized, advanced toward the incision and precisely removed. Since no two faces are the same, the degree of each procedure is uniquely tailored for each individual. The result is a contoured neck and jawline with smoother and firmer skin. Predictably your face will have a rejuvenated and refreshed appearance without the unnatural, operated stigmata.

“Dr Fink did an incredible job!…Being kind of awake, it was nice to be aware and feel the care the doctor took in each section of my face! My stitches come out next week and I have very little bruising! Very little pain… I am excited, so far everything looks great!”

Who is a candidate for a DeNovo lift?

Aging has much to do with genetics and lifestyle. People of certain ethnicities may be more resistant to signs of aging than others. This often correlates with the degree of skin pigment since it protects from the harmful UV rays of the sun. For example, people of Western European heritage will show signs of aging earlier than African or Asian Heritage. However, the effects of lifestyle factors like smoking have been demonstrated in studies of identical twins. There is no specific age minimum or limit for having the procedure. The time for a procedure is determined by your visable aging and your tolerance of it. When you look in the mirror or see yourself in photos, you may notice jowling, wrinkling or a turkey neck. If it improves with lifting the soft tissues, you may well be a candidate for the DeNovo lift. Once the tissues are lifted and excess skin removed, it will not grow back. As you get visably older, you will still appear younger than had you not done anything. The appearance of your aging will be turned back, but it will continue to progress according to your genetics. It may be slowed, however by good skin hygiene, maintenance procedures and lifestyle choices.

How much does a DeNovo Lift cost?

A DeNovo lift is performed in a surgical center under local anesthesia. Cost of the procedure is generally more affordable than a traditional facelift under general anesthesia. Some of the cost is determined by the specific needs of each patient and the time involved. For example, a patient may perform additional enhancement procedures at the same time for a better result. This also permits a better cost value in most cases. The DeNovo lift is affordable for most working and retired adults. Call today for a complimentary consultation and customized quote.

Who is not a candidate for a DeNovo lift?

Anyone considering any type of facelift or mini lift should consider the DeNovo Lift. Candidates for the DeNovo lift include anyone who is healthy with normal patterns of aging. Athough there are no routine lab tests required, a medical clearance from your physician may be requested in some cases. Since it is done under local anesthesia, it avoids many of the concerns posed by general anesthesia. Nevertheless, all procedures have associated risks. In most cases, preexisting health conditions pose the greatest risk to an uncomplicated recovery with the DeNovo lift. Dr Fink may consult with your established physician to assist in the assessment.

Typically people from Western european descent will see significant changes to their facial skin texture by age 40. Skin elasticity generally decreases over time with loss of collagen. Smart Lipo helps increases collagen to strengthen the skin. Loose support muscles are firmed beneath the skin and fatty deposits are countoured. Lastly, the skin is custom fitted to provide a younger appearance allowing natural movement. If you are in good health, age is not a contraindication to the procedure. Dr. Stephen Fink has performed the procedure on patients up to ninety years old. It can also be performed as a refresher to a previous facelift. Combined with CO2 laser resurfacing, the skin texture of the entire face can be enhanced to produce a synergistic effect lessening wrinkles in areas not addressed by a facelift alone. Whether you have a small amount of loose skin about your neck or a heavy neck with significant jowling, Dr. Stephen Fink will adjust the procedure to achieve the best possible results for each individual.

What can I expect on the day of the procedure?

On the day of the procedure, you are advised to eat a light meal before arriving at the office. Oral medications prescribed at your preop visit are brought to the office and given to achieve a relaxed state. At this point, a dilute numbing solution of local anesthesia will be injected into the face and neck with minimal discomfort. You may get sleepy or even nap during the procedure but you will remain arousable and able to breath on your own. There is no need for a breathing tube or ventilator. You can listen to your favorite music during the procedure or have a collegial conversation with Dr. Fink which can make the experience memorable. Once the procedure is finished, a supportive dressing is placed and you can go home shortly after.

Undergoing a face and neck lift under local anesthesia allows your body to remain physiological stable during the procedure. This reduces bruising, swelling and incidence of hematoma formation (a collection of blood that occurs under the skin after the procedure).

What can I expect in terms of recovery?

Traditional deep plane facelifts require several weeks to recover. Swelling and bruising is often quite severe. The DeNovo lift minimizes bruising though some should still be expected. Swelling is variable but is less in most cases. Prepare for surgery by eliminating blood thinners and supplements from your diet 1-2 weeks before surgery. The herbal supplement Arnica Montana is recommended prior to surgery as it is widely believed to reduce bruising and swelling. Regular icing along with minimal activites during the first several days afterward has been correlated with less initial bruising. Small sutures that are placed at the hairline and around the ear are removed in 7-8 days. This is the recommeded time off work in the majority of cases. Any residual bruising can usually be covered with make up at that point. You may be feeling great by then and you can certainly be out in public depending on your comfort level with your appearance. You are advised to continue limiting activity to lifting no more than 10-15 pounds and to avoid activities that raise heart rate or blood pressure for the next 2 weeks. With normal healing there are generally no restrictions after that time.
