What is a DeNovo Lift?
The Denovo lift is a hybrid facelift procedure combining the safety of local anesthesia with advanced laser technology. It is customized to each individual and when done in combination with other rejuvenation procedures, provides a graduated approach to facial rejuvenation. Over the years, the traditional facelift procedure has evolved into what is now known as a deep plane facelift. In recent years, “mini-facelift” procedures have seen popularity in the office setting. Most of these procedures involve smaller incisions with limited tightening in the front of the ears. While these procedures provide an adequate results for the right candidate, they often provide minimal results for fullness or significant laxity in the front of the neck. The best candidates for good results are either younger patients without significant neck concerns or a patient desiring a refresher for a previous facelift.

The DeNovo lift is a synthesis of proven techniques including laser liposuction. At the start of the procedure, a laser fiber is gently passed right beneath the skin in several directions to help disrupt fat cells, lessen bleeding and shrink the skin. This helps reduce bruising and provides additional tightening and thickening of the skin as it stimulates the growth of new collagen. Traditional liposuction is then performed to the desired contour followed by a corset style platysmaplasty. Who is a candidate for a DeNovo lift?
Aging has much to do with genetics and lifestyle. People of certain ethnicities may be more resistant to signs of aging than others. This often correlates with the degree of skin pigment since it protects from the harmful UV rays of the sun. For example, people of Western European heritage will show signs of aging earlier than African or Asian Heritage. However, the effects of lifestyle factors like smoking have been demonstrated in studies of identical twins. There is no specific age minimum or limit for having the procedure. The time for a procedure is determined by your visable aging and your tolerance of it. When you look in the mirror or see yourself in photos, you may notice jowling, wrinkling or a turkey neck. Who is not a candidate for a DeNovo lift?
Anyone considering any type of facelift or mini lift should consider the DeNovo Lift. Candidates for the DeNovo lift include anyone who is healthy with normal patterns of aging. Athough there are no routine lab tests required, a medical clearance from your physician may be requested in some cases. Since it is done under local anesthesia, it avoids many of the concerns posed by general anesthesia. Nevertheless, all procedures have associated risks. Typically people from Western european descent will see significant changes to their facial skin texture by age 40. Skin elasticity generally decreases over time with loss of collagen. What can I expect on the day of the procedure?
On the day of the procedure, you are advised to eat a light meal before arriving at the office. Oral medications prescribed at your preop visit are brought to the office and given to achieve a relaxed state. At this point, a dilute numbing solution of local anesthesia will be injected into the face and neck with minimal discomfort. You may get sleepy or even nap during the procedure but you will remain arousable and able to breath on your own. There is no need for a breathing tube or ventilator. You can listen to your favorite music during the procedure or have a collegial conversation with Dr. Fink which can make the experience memorable. Once the procedure is finished, a supportive dressing is placed and you can go home shortly after.
Undergoing a face and neck lift under local anesthesia allows your body to remain physiological stable during the procedure. This reduces bruising, swelling and incidence of hematoma formation (a collection of blood that occurs under the skin after the procedure).
What can I expect in terms of recovery?
Traditional deep plane facelifts require several weeks to recover. Swelling and bruising is often quite severe. The DeNovo lift minimizes bruising though some should still be expected. Swelling is variable but is less in most cases. Prepare for surgery by eliminating blood thinners and supplements from your diet 1-2 weeks before surgery. The herbal supplement Arnica Montana is recommended prior to surgery as it is widely believed to reduce bruising and swelling. Regular icing along with minimal activites during the first several days afterward has been correlated with less initial bruising. Small sutures that are placed at the hairline and around the ear are removed in 7-8 days. This is the recommeded time off work in the majority of cases. Any residual bruising can usually be covered with make up at that point. You may be feeling great by then and you can certainly be out in public depending on your comfort level with your appearance. You are advised to continue limiting activity to lifting no more than 10-15 pounds and to avoid activities that raise heart rate or blood pressure for the next 2 weeks. With normal healing there are generally no restrictions after that time.
